This template is meant to be used on the image description pages for images of female models. It generates a table with some biographical and size information about the model in the picture.
The main use for this is when you wish to describe the character that you are using the model as a depiction of. For example, you wish to describe her height or weight for context or for a plot point. Rather than having to pull numbers out of thin air, which may or may not even make sense for the woman pictured, you can just use the actual model's height or weight as the basis for the character's height or weight.
Usage
The template's basic usage is like this:

All elements are free-form - you can fill them out any way you please.
